Basic Policy on the Protection of Personal Information

PhoneAppli Co., Ltd. (hereinafter referred to as "the Company") fully acknowledges the social mission regarding the protection of all personal information managed by the Company, and shall observe laws and regulations concerning personal information. Furthermore, we have established a personal information protection management system to embody this policy, and here we declare that, while always striving to be aware of the latest trends in IT technology, changes in social demands and business environment, etc., we will work on a company-wide scale to continuously improve the management system.

Personal information shall be obtained, used, and provided only to the extent necessary for the Company's legitimate business execution, employment of staff members, and personnel management as written in the following items, and we shall not use personal information beyond the scope necessary for achieving the specified purpose of use (use other than the intended purposes). We will also take measures to prevent use other than the intended purposes.

Furthermore, in the case of emergency, we will promptly take corrective actions.
  4. 4. Toward grievances and inquiries concerning handling of personal information, we will respond promptly, sincerely, and appropriately.
  5. 5. Based on changes in the environment surrounding the Company, we will revise our personal information protection management system in a timely and appropriate manner, and continuously promote its improvement.

We will not only notify all our employees of this policy by distributing it to them but also take measures to enable anyone to be able to obtain it at any time by posting it on our website, pamphlets, etc.

Regarding Handling of Personal Information


Purpose of use of personal information obtained by the Company

Regarding personal information subject to disclosure maintained by the Company, requests from individuals in question or their representatives, concerning such matters as notification of the purpose of use, disclosure, correction, addition or deletion, suspension of use, elimination, or suspension of provision to a third party (hereinafter referred to as "request for disclosure, etc."), we will respond in the following manner.

f) Procedure of responding to requests for disclosure, etc. of personal information subject to disclosure

1) Contact to request disclosure, etc.
For requests for disclosure, etc., please contact the Personal Information Inquiry Desk mentioned above.

2) Procedure concerning requests for disclosure, etc.
i) After receiving a request, we will e-mail the individual the standard request form, "Request for Disclosure, Etc. of Personal Information Subject to Disclosure" to be filled out.
ii) The individual should send, by post, the filled-out request form, the document that attests it is the legitimate representative if the request is made by a representative, and the postal money order for the fee (only in cases of notice of use purpose and request for disclosure) to the Personal Information Inquiry Desk.
iii) After receiving the above request form, in order to confirm the identity of the person making the request, we will contact the individual to cross-check a number of items of information (such as telephone number, date of birth, etc.) with them against information already registered with the Company.
iv) In principle, replies will be sent in writing (enveloped postal mail) to the individual in question.

3) Document that attests it is the legitimate representative if the request is made by a representative
If the person making the request disclosure etc. is a representative, please enclose a document that proves that the person is a legitimate representative, and an identification document of the representative. Regarding the registered domicile contained in each material, please show only the name of the prefecture, and black out or use another method to hide the rest of the address information. In addition, documents to be sent should not include the "Individual Number", or if they contain the "Individual Number", black it out or use another method to hide it.
